At Lot Five Studios, your satisfaction is always our goal. We want you to love your child's school portraits. Fill out this form if your student was photographed and you are unsatisfied with the image. If you'd like us to try again, give us some notes on how we can make it better for you. ABSENT AND/OR NEW STUDENTS WILL BE PHOTOGRAPHED AUTOMATICALLY FOR YEARBOOK PORTRAITS.

Please don't forget to inform your child that they will be photographed again on Retake Day and to give us their best smiles!
